Korzystanie z trombolitikov jest przeciwwskazane u pacjentów z aktywnym krwawienie (wewnętrzne), uraz/operacja (niedawny PPC/udar/chirurgii kręgosłupa w ciągu 2 miesięcy), diateza krwawienia, historia choroby naczyń mózgowych (cv) wypadku, wewnątrzczaszkowe defekt (tętniaki, wady rozwojowe tętniczo, guzy), lub ciężkie niekontrolowane nadciśnienie tętnicze (ciśnienie skurczowe>180/DBP>110). Ryzyko i korzyści powinny być dokładnie rozważone w takich warunkach i leczenia trombolitycznego jest podawany z ostrożnością u pacjentów z niedawnym (10 dni) poważne krwawienie z przewodu pokarmowego lub niedawny (10 dni) operacja chirurgiczna (pomostowania, ginekologiczno wysyłka, organ biopsji, nakłucia naczyń, nieuchronnie), lewicowych działów serca, skrzepliny, podostre bakteryjne zapalenie wsierdzia, defekt hemostazy, KOL choroby, cukrzycowa retinopatia krwotoczna, ciąża. Obserwacje kliniczne krwi, krwawienia i krzepnięcia zalecane przed rozpoczęciem leczenia trombolitycznego. Wskaźniki aktywność fibrynolityczna i/lub funkcji krzepnięcia krwi podczas infuzji nie korelują z efektywnością i ryzykiem krwawienia.
